This form must be submitted within 24 hours of booking a training session. If you fail to do so your time slot may be forfeited. 

 

Your $50 deposit is non-refundable and will contribute to the total cost of your training session. The remaining balance is due the day of your session via cash, check, or Venmo. In the unfortunate event that something comes up and you need to reschedule or cancel your appointment please let me know within 24-48 hours PRIOR to your session.

Any request to reschedule your session needs to be made within this time frame or your deposit will be forfeited.
